With uncertainty in the health care environment and insurance benefits changing annually, LTPT will help each client determine what insurance benefits are available on a case by case basis. LPTP is a registered in network provider with Medicare. In order to maintain the high standards of care for our clients, LTPT works out of network with most private insurance providers. If requested, LTPT can contact your provider to help determine what out of network benefits are available to you. Over the years we have developed the expertise in obtaining all necessary information from the insurance carriers regarding out of network benefits. In addition, LTPT can also submit all claims electronically in order to expedite reimbursement.

First Time Patients

• Bring your insurance card and prescription.
• Come 10 minutes early to fill out paperwork, or go to our Forms section, down and complete appropriate paper work in advance.
• Bring comfortable gym clothes including socks. No special foot wear or sneakers are required. In most cases as you will be working bear foot or in socks.

Cancellation

Please give us a ‘One Day’ notice for cancellations. Late cancellations and ‘No Shows’ are subject to a $160 charge. Call if you have a question or a concern or if you will be late. We are sensitive to the complexities of life in New York and implement this cancellation policy with understanding and sensitivity. However, we need to know about cancellations in advance for the consideration of other patients and clients.
